Safeguarding Adult Review in respect of Alice and Peter

The North Yorkshire Safeguarding Adults Board (NYSAB) has accepted the findings and recommendations of a Safeguarding Adult Review (SAR) it commissioned with regard to ‘Alice & Peter’.

‘Alice & Peter’ were a couple in their seventies, living together in a flat in North Yorkshire. They both sadly died in January 2023, days apart with their cause of death recorded as natural causes, however Alice & Peter were found to have been living in circumstances of hoarding and self-neglect. Alice had been well known to numerous agencies within North Yorkshire and although Peter was not well known to professionals, he was identified as needing support after Alice’s death.

The review seeks to understand the issues that prevented those involved from being able to properly help and protect ‘Alice & Peter’ from harm. The SAR provides recommendations which aim to support good working practices for the future.

The purpose of a Safeguarding Adult Review is to look at how multiple agencies can best work together to protect people. It provides recommendations on how lessons can be learned from their experiences to improve practice and services.

North Yorkshire Safeguarding Adults Board offer their sincere condolences to Alice & Peter’s family and friends.
