The North Yorkshire Safeguarding Adults Board (NYSAB) has accepted the findings and recommendations of a Safeguarding Adult Review (SAR) it commissioned with regard to ‘Courtney’.

‘Courtney’ is a young woman living in North Yorkshire, with care and support needs and undiagnosed learning disability. A report was commissioned to consider how agencies of the Safeguarding Adults Board worked together to keep her safe in a time period between the loss of her first child at birth, and the birth of her second child.

The review seeks to understand the issues that prevented those involved from being able to properly assess ‘Courtney’ and offer appropriate support for her needs. The SAR provides recommendations which aim to support good working practices for the future.

The purpose of a Safeguarding Adult Review is to look at how multiple agencies can best work together to protect people. It provides recommendations on how lessons can be learned from their experiences to improve practice and services.
