Safeguarding adult review policy
The existing NYSAB safeguarding adult review policy has been reviewed and updated to ensure it is effective, compliant, and reflective of best practice. Updates have been drafted following discussion with Independent Chair and the Business Unit, and through reviewing SAR policies from East Riding, York, Teeswide, Bradford and Stoke and Staffordshire SABs.
The Learning and Review sub-group reviewed an initial draft on 16th January 2025. Further feedback from this group was collated to inform the final draft presented to the Board.
Main changes to safeguarding adult review policy
•Language – additional clarity around Section 44 criteria and updating of language. Cease referring to ‘Learning Events’, ‘Discretionary SARs’, ‘non-Stat SARs’ ‘Roundtables’, etc.
•Structure – re-written in line with new subgroup structure, separating SARs out of the current LAR to ensure dedication of time, focus and monitoring.
•Forms –updated referral and decision-making form, to assist referrers and scoping panels in the process and improve recording.
•Chronologies – Moving from collation of full chronologies prior to scoping, to a request for initial information that is appropriate / proportionate (with robust analysis e.g. around service gaps, missed opportunities, good practice) in order to make a decision whether a referral meetings the S44 criteria. This should reduce partners workloads, in particular for referrals which are then deemed not to meet the SAR criteria. If fuller chronology information is needed following commissioning of a SAR, this will be actioned.
•Decision making / Voting – a clearer processes, ensuring a fair and robust voting supported as above with better recording.
•Accountability – clarifying processes for ongoing review and sign off of action plans
